When Visiting Us for the First Time
Please bring a valid photo ID such as a health insurance card or driver’s license for identity verification. If we are unable to confirm your identity, we may not be able to proceed with your consultation.
Before Receiving Treatment
You will be required to sign a consent form before undergoing any procedure. Consent is provided via digital signature, so there is no need to bring a personal seal (hanko).
If Anesthesia Is Used
For procedures involving nitrous oxide anesthesia, intravenous anesthesia, or local anesthesia on your arms or legs, we kindly ask that you do not drive yourself home (car, motorcycle, or bicycle). If you have arrived by car or other vehicle, please arrange for alternative transportation, such as a driving service, after your treatment.
